 PSR Information and Forms
In response to increased demand by the courts for Pre-Sentence Reports (PSRs) with psychological/psychiatric components, and to changes related to the “Blackwell Memorandum of Understanding” signed in November 2008, the following information and forms are provided to assist in ordering or requesting PSRs with Psychological/Psychiatric Assessments:
- FPSC General Referral form (Word, 107KB)
This form has been updated to allow selection of an ‘ordered' PSR versus a ‘requested’ PSR. The form requires specific information as to why the PSR is ordered, to assist clinicians in providing reports which better meet the needs of the court at the time of sentencing.
- PSR Referral Information Checklist (PDF, 9KB)
The checklist outlines necessary referral information to be provided by each agency.
- The following documents are excerpts from the Blackwell MOU which outline the responsibilities of the respective partner agencies, including necessary materials and timelines for completion of PSRs:
