Admission to the ADHD Program requires a written referral from a family doctor, pediatrician or psychiatrist. You can request a referral form by calling 604-875-2010 or downloading the form here. Along with medical history, we require the results of a recent physical examination. Once we receive the completed referral form, the referral is reviewed by our team. If it is accepted, we contact the patient or family directly to set up a first appointment. Following our assessment, we send a report back to the referring physician.
