 How are we doing

Providing Feedback

Service Feedback/Patient Concerns Forms
If you have a comment, an issue, or a compliment about any of our services, one way to share that with us is to fill out a Service Feedback form (for Riverview Hospital) or a Patient Concerns form (for Forensic Psychiatric Services). Copies are available on all patient units and information desks at the two hospitals, and also at each Forensic Regional Clinic. RVH feedback forms are also available at Pennington Hall and the Library in the HEY Building. Please ask patient/client care staff if you need assistance getting a feedback form.

Motivation, Power & Achievement (MPA)
Another avenue for feedback at Riverview is the hospital-wide advocacy program run by the MPA (Motivation, Power & Achievement) Society. The program focuses on care and treatment issues raised by patients and residents, their families and others. On-site program advocates help you act on your own behalf, or advocate on your behalf. They can be reached at 604-524-7874.

Patient Care Quality Office

Effective October 2008, each of BC's six health authorities (including PHSA) has a centralized Patient Care Quality Office. Patients or family members who have concerns about their experience are encouraged first to resolve the issue with the hospital or clinic, and if still unsatisfied, to contact the Patient Care Quality Office. The office registers all complaints and tracks them through the review process to ensure a timely response. If you feel your concern is still not resolved by the Patient Care Quality Office, you can take it to a review board for consideration of an independent review.

The PHSA Patient Care Quality Office is located at BC Children's Hospital, 4500 Oak Street, Vancouver. Call toll-free, 1-888-875-3256, or fax 604-875-3813, or email pcqo@phsa.ca. Their website is www.phsa.ca/PatientCareOffice
